DescriptionA framed Slovak Roman Catholic document noting the birth and baptism of Adolf Skvarek on March 17 and 28, 1918, respectively. The word "Extractus" is printed across its top in bold lettering. The entire document is in Latin with dates and Skvarek's information handwritten in the chart. A date and signature appear at the bottom, along with two official seals. The name Adolf has been handwritten in pencil at the bottom left cornerof the document.
